The guy that has written Failbot has quit raiding and is going to quit the game entirely as well so this addon is no longer being continued. Luckily there is already a replacement available called EnsidiaFails : http://wow.curse.com/downloads/wow-addo .... fails.aspx
The latest version seems to have quite some fails regarding CotC already and some additions to Ulduar as well
